It’s easy to apply for the Massage Therapy Program

Simply complete steps 1 – 7.

  1. Step 1: Download and complete the application form.

  2. Step 2: Download the Myotherapy Institute health form and have your physician mail the completed form to us.

  3. Step 3: Download and have two references complete the referral form. One professional and one personal. Relatives are not eligible to submit recommendations.

  4. Step 4: Submit a photocopy of the student’s driver’s license with recognizable photograph.

  5. Step 5: Send an official high school transcript or GED mailed by the school attended.

  6. Step 6: Send a non-refundable $150 application fee.

  7. Step 7: Complete a personal interview after steps one through six are processed.

Application materials will be reviewed for acceptance when steps one through seven are complete. A letter of denial or an acceptance letter with class schedule and registration form will then be mailed.

Applications for the Massage Therapy Program are accepted throughout the year. Introductory classes begin each August and January.
